Preparing for Discharge
Our case management department assists patients through a hospitalization, whether it is acute care or swing bed, to ensure all available resources coordinate toward a patient's recovery.
Our case manager collaborates with the entire professional health care team.
Services can include:
- Therapy referrals
- Community resources after a hospital stay, such as 60s Plus Dining, homemaking/bathing services or finding support groups
- Arranging needed home equipment
- Easing transitions to post-hospital services such as nursing home, assisted-living, skilled home health, cardiac rehabilitation, or hospice
Our caring staff assists our patients and their families who may experience a difficult, stressful, or even life-changing event.
