MyChart Patient Portal

Brookings Health's patient portal allows you to securely access your health information and interact with your care team. In MyChart, you can:

  • Connect care visits and medical history from multiple providers, including Avera and Sanford.
  • Schedule care easily online or in the app.
  • Connect with providers with virtual visits.
  • Manage care for family members.
  • Pay premiums and see medical bills.
  • See current prescriptions and request refills.

Login to MyChart

To access your MyChart, sign in with your login credentials below and you will be routed to the MyChart home page.

Sign Up for MyChart

Current patients and new patients will need to create a MyChart account. Your old AveraChart account login and password will not transfer to the new patient portal.

If you’re opted in to receive emails or texts from Avera, you may receive a message to sign up for MyChart.

Proxy Access in MyChart

As of May 30, 2026, if you had access to another person’s health information for coordinating care, you need to reestablish proxy access in MyChart. Common proxy relationships include parents and children, or if you help manage your parent or another adult’s health appointments.
